Toolkit for improving chronic illness care launched

The US Government’s Agency for Healthcare Research and Quality (AHRQ) has issued a toolkit for improving care for the chronically ill. The toolkit is specifically aimed at ‘safety net’ providers to provide system redesign help following the Chronic Care Model. The toolkit covers specific practice changes involved in chronic care model implementation, integrates business strategies to address the financial and operational barriers to quality improvement, links over sixty tools commonly used for quality improvement and includes case studies.
